During this webcast we examined the White Earth Nation Coordination, Assessment, Resource and Education (WE CARE) case management model which has received national attention for its innovative approach to service delivery in the wraparound program.
Using community outreach, the White Earth Nation discovered that members were unaware of many of the programs and services that were available to members, and when they did access services, members were often confused or overwhelmed by the lack of collaboration between programs.
Next, the White Earth Nation developed the WE CARE Program to ensure that programs work together to provide wraparound services for the entire family.
WE CARE operates from a RiteTrack database system that provides an efficient and effective way to link families to the services they need.
The WE CARE model has been successful in reducing duplication of services, enabling better communication and coordination, and increasing referrals and utilization of services and resources.
RiteTrack supports the goals of the WE CARE program by enabling assessment, collaboration, plan development, and outcome tracking for the White Earth Nation.
